Enroll in School with our Registrars

INFORMATION NECESSARY FOR ENROLLMENT:

1. Last report card and/or transcript
2. Withdrawal form (during school year)
3. Proof of address - utility bill (electric, water or gas)
4. Copy of birth certificate
5. Copy of social security card
6. Current immunization record
7. Legal guardianship papers if not living with parent

IF THE ABOVE INFORMATION IS NOT COMPLETE, ENROLLMENT
WILL BE DELAYED UNTIL ALL INFORMATION IS PROVIDED.
