Transcript Requests

Current Students: Please fill out the request form in the counseling office.

Graduates Only: Official transcripts can only be mailed to the college or institution and cannot be mailed to the parent or student.

  • Request can NOT be taken over the phone.
  • Request can be faxed to Shelley Cope, Registrar at 972-436-7408
  • Request can be mailed to Lewisville High School, 1098 W. Main St, Lewisville, TX 75067
  • Request can be e-mail to copes@lisd.net
  • Please provide the following information in your request:
    1. Student's Name
    2. Year of Graduation
    3. Birth Date
    4. Name of college or institution
    5. Address of college or institution
